I used to clean my house's floors, and then I became a wizard, and now someone cleans my floors for me because if I spend my time cleaning my floors then I don't have that time to study magic and then demons will invade and eat everyone. You have advanced medical knowledge, and now someone should clean your floors for you, because if you waste your time cleaning floors then babies will die. So what I think is that you should call for a servant and ask for them to fetch you whatever you want, and then I am going to try to get it into your head that you are important and should act like you're important, and then if you manage to grasp this concept before the food gets here you can fuck my ass, as a reward, because people learn things better with rewards and because how much I want you to fuck me is in fact totally immaterial compared to you learning your place in the world because you are more important than I am."

...why does that "I love" make him feel sick and gross inside. He wouldn't be saying it if not for the Charm Person but that's just normal. Does Tanidio somehow expect that someone who isn't his mom is going to say "I love you" to him unCharmed? It'd just trade off against using it for sex, but Bear's Endurance and the amount of magical healing I've received for the last five years ought to be enough for that unless you have very unusual tastes. Since at baseline you can do things to me that would kill most peasants."
